Spartacus Consulting offers vision coverage through Dental Select (don’t be fooled by the company name, this is the vision carrier). You are able to sign up for vision coverage without enrolling in health coverage.
REQUIRED FORMS
To sign up for or to waive vision coverage, please use the same employee election form that you use for your health coverage.
You’ll also need to complete the Dental Select enrollment form.

The Choice Vision 8 plan through Dental Select pays 100% of the cost of for in-network vision exams once every 12 months and provides an allowance for frames, lenses, and contacts.
