To sign up for or to waive dental coverage, please use the same employee election form and BCBSTX enrollment form that you use for your health coverage.
PLAN OPTIONS AND MONTHLY RATES
The DTXHR01 dental plan through Blue Cross Blue Shield of Texas pays 100% of the cost of preventive care, 80% of the cost of basic services, and 50% of the cost of major services with a $25 individual deductible and $75 family deductible and an annual cap of $3,000.
